The Tooth Fixer In-House Dental Plan is designed specifically for our patients who do not have dental insurance, but still want to maintain optimum dental health at a reasonable cost.
The Tooth Fixer Family Dental Care In-house Membership Plan is NOT dental insurance. It is also not a dental discount plan. In many ways our plan is actually better than most Dental Insurance. No deductibles, no annual maximum benefit amount, no waiting periods. (Composite fillings, cosmetic treatment and whitening all included!)
What's Included?
Two preventive dental exams, including necessary x-rays, routine cleanings, periodontal evaluation, medical history review, blood pressure screening, oral cancer screening, and fluoride treatment (up to $676 value).
Members will be eligible for a 17% discount off our standard fees for any additional exams and dental treatment performed at The Tooth Fixer Family Dental Care for one full year when payment is made in full at time of treatment.
The "Fine Print"
The term of membership is one full year from the date of enrollment (ie. August 7, 2017- August 6, 2018). Patients may choose to enroll at the time of their preventive exam, or within 30 days of that exam and the start date will be adjusted to the date of prior exam.
The cost of membership is $327. This fee is non-refundable. Membership is not transferrable. Payment in full and a completed membership form must be submitted upon enrollment. Call or ask
The six-month periodic exam and cleaning must be completed before the end of the 7th month of enrollment or it will no longer be included with the current year’s membership.
Notification for the cancellation or rescheduling of a scheduled six-month periodic exam must be given no less than 72 hours (3 days) prior to the scheduled appointment time or it will no longer be included with the current year’s membership.
The 17% discount applies solely to dental treatment at The Tooth Fixer Family Dental Care, performed during the term of membership, and does not apply to any treatment performed by any other dental practice(s) or specialist(s).
The 17% discount of will be applied only to treatment paid in full by cash, check, credit card, HSA card or debit card at the time of treatment. No discount will be applied for any treatment paid for using an outside financing company such as CareCredit, etc.
The 17% discount cannot be combined with any existing specials or promotions. However, the lowest price of the two will prevail.
